Delayed-Onset Hepatic Encephalopathy Induced by Zolpidem: A Case Report
نویسندگان
چکیده
Hepatic encephalopathy (HE) is a complex neuropsychiatric disorder associated with either hepatocellular insufficiency and/or the presence of porto-systemic shunts. It is thought to be induced by altered neurotransmission mediated by enhanced gamma-aminobutyric acid (GABA)-induced inhibitory tone. The use of benzodiazepines are generally avoided in patients with advanced chronic liver disease (CLD) because they enhance GABA-mediated neurotransmission. Zolpidem is a non-benzodiazepine hypnotic that selectively binds to the omega-1 GABA-benzodiazepine receptor complex. Although it theoretically has the same propensity to induce HE, its shorter half-life when compared to commonly prescribed benzodiazepines like diazepam, alprazolam and clonazepan, makes it a reasonable choice for subjects with CLD. We report a patient with CLD with a rapidly induced grade IV HE attributed to zolpidem that was promptly reversed by flumazenil.

Treatment of Ifosfamide -induced Encephalopathy with Methylene Blue in an Adolescent with Relapsed Acute Lymphoblastic Leukemia: a case report
Ifosfamide is one of the most important chemotherapeutic agents used for the treatment of numerous types of malignancies. Neurotoxicity is one of its life-threatening complications and is potentially fatal. In this paper we present a case of severe ifosfamide- induced encephalopathy in an adolescent with acute lymphoblastic leukemia in whom treatment with methylene blue led to complete improvem...
متن کاملEvaluation of the role of TLR4 in endotoxin-induced hepatic encephalopathy in rats with biliary cirrhosis
Background: Hepatic encephalopathy is defined as a neuropsychiatric brain dysfunction in acute or chronic liver failure. Infection and inflammation have crucial role in its pathophysiology. The purpose of our study was to demonstrate the relationship between toll-like receptor 4 (TLR4) expression and the encephalopathy induced with endotoxin in biliary cirrhotic rats. Methods: The present stud...
متن کاملBilateral C5 Palsy Following a Circumferential Surgery for Cervical Spondylotic Myelopathy: Case Report and Review (In press)
Background and Importance: The development of C5 root palsy is a well-known potential complication of cervical spine surgeries for the correction of cervical spondylotic myelopathy. It typically occurs unilaterally but on extremely rare occasions it might occur as a bilateral anomaly. The value of intraoperative neurophysiologic monitoring in detection of iatrogenic acute versus delayed onset C...
متن کاملCauses of Liver Dysfunction in Critical Illness
ACUTE LIVER FAILURE Acute hepatitis with jaundice and coagulopathy without hepatic encephalopathy is referred to as severe acute hepatitis. The prognosis is generally good. The term fulminant hepatic failure (FHF) was originally defi ned as a clinical syndrome developing as a result of massive necrosis of liver cells or any other sudden and severe impairment of hepatic function (Trey and Davids...
متن کاملA New Nonsense Mutation in CDKL5 Gene in a Male Patient with Early Onset Refractory Epilepsy: a Case Report
Background The X-linked cyclin-dependent kinase like 5 (CDKL5/STK9) gene has been shown to be responsible for a severe encephalopathy condition characterized by early onset of epilepsy and severe developmental delay. CDKL5 mutations have been shown to be more frequent among female patients. Results Here we report a 6- month male patient, second child of a healthy non consanguineous in the Irani...
